3. Endorsement in Driving Licence to drive any other class of vehicle i. Completion of 18 years for Motor Cycle withgear & Light Motor Vehicle
ii. Completion of 20 years for Transport Vehicle with 1 year experience in LMV & Badge.
iii. Applicant should appear in person. 1. Form 8
2. Form 5
(Form 5 only for those who studied through Driving School) 1. Driving Licence.

4. Consent letter from vehicle owner, to conduct driving test in his vehicle, if the vehicle is not owned by the applicant. 4. Public Service Badge i. Completion of 20 years

5. Endorsement to drive Hazardous goods vehicle 1. Should have a Driving Licence to drive Transport Vehicle.
2. Training Certificate from I.R.T. and other recognised institutions.

7. Renewal of Driving Licence 1. Holder of driving Licence

3. Re-test and renewal when D.L.Expires more than five years 1. Form-9

(1a-if the applicant is above 40 years 1. Driving Licence
2. Age Proof
3. Passport Size photos 3 for book type and 2 for laminated licence i. Rs.55 for laminated type
ii. Compounding fee for belated renewal Rs.10 per year.
